This lecture will review the surgical management of the diabetic foot in the setting of gangrene and tissue loss, emphasizing decision-making across the spectrum from urgent amputation to advanced limb salvage techniques. Participants will examine indications for surgical intervention, principles guiding the timing of debridement and amputation, and factors influencing the level of amputation to optimize healing and function. The session will also highlight contemporary surgical strategies for limb preservation, including reconstructive and external fixation (“X-Fix”) approaches, with the goal of improving outcomes in patients with complex diabetic foot pathology.
